The goal of a root canal treatment

Only healthy teeth can remain in the oral cavity long term. If the root canal system is infected with bacteria, it can lead to a severe toothache and/or inflammation (periodontitis apicalis) around the apex of the root. Endodontics is the field of dentistry concerned with the interior of teeth, the root canal system. The main goal of endodontic therapy is to eliminate bacteria from the entire root canal system by way of disinfection as well as to remove all residual tissue from the canals. With the help of modern endodontics, tooth nerves that are irreversibly inflamed or that are already dead can be rescued in the long term and serve as a foundation for crowns, bridges or other prosthetic replacements.

Working with the surgical operating microscope

With the help of the 25x magnification of the surgical microscope, the root canal system can be inspected better and therefore treated better. The root canal length is determined by measuring the electrical impedance during root canal treatment. The surgical operating microscope ensures optimal visibility, so that the anatomy of the affected tooth can be treated individually while being gentle on the tooth structure at the same time. Using ultrasonically activated irrigation for the cleaning of the root canal system, mechanical preparation of the root canal system using hand instruments, as well as various nickel-titanium files, the root canal system is hermetically sealed and the tooth is stabilized with the help of a post-endodontic adhesive crown core buildup until the final restoration.

Endodontic revision

Root canal revision (retreatment) may become necessary for teeth that have already had endodontic treatment. Common causes for revisions are toothaches, inflammation at the apex of the root of the affected tooth (periodontitis apicalis) or inadequate root canal treatment that must be remedied before prosthetic replacement (e.g., crown or bridge).

During revision treatment, the old root canal filling material is removed (revised) and the entire canal system disinfected with the help of a surgical operating microscope. Furthermore, the optical magnification instrument is useful in determining whether there are any other untreated root canals that may cause pain or inflammation at the apex of the root. By working "under view" with the help of the surgical operating microscope, it has become possible to close perforations (non-physiological holes in the tooth) or open apexes with a special cement (MTA). This way, hopeless teeth may possibly be saved after all.
